Cook, Tom Passed away peacefully on 17th February 2019 aged 70 years. A devoted husband and father. Missed by Christine, Daniel, Kieran, Sally and Hannah The funeral will be held on Wednesday 6th March 2019 at 11.00am at The Chilterns Crematorium, Amersham.
